Characteristics of Schizophrenic Patients with Alcohol Abuse

Abstract
Objective£ºAlcohol abuse and dependence complicate the diagnosis and management of schizophrenics. Up to 60% of chronic schizophrenic patients are reported to abuse alcohol or drugs. Moreover alcohol misuse are associated to increased risk of relapse and rehospitalization, decreased treatment response and reduced functional status. This study aimed to compare schizophrenics showing hazardous drinking with schizophrenics without hazardous drinking and define various characteristics of them.

Method£ºAll 76 schizophrenics were admitted to Seoul National Mental Hospital for eight months (May through December 1998). The AUDIT(Alcohol use disorder identification test) was used to evaluate their current alcohol use. We divided the all subjects by AUDIT score(8 or more AUDIT score versus AUDIT score below 8) and compared both groups about clinical characteristics.

Result£ºThere are no significant demographic differences between two groups. There are no significant differences in admission frequencies and duration of hospitalization, but GAF score is significantly lower in hazardous drinking group. Family history of alcoholism and paranoid type of schizophrenia are significantly higher in hazardous drinking group. Comparing the PANSS score between two groups, the positive symptom scale is higher and the negative symptom scale is lower in hazardous drinking group. In the general psychopathology scale, the poor impulse control scale is significantly higher in hazardous drinking group and significantly correlate with AUDIT score. The degree of poor impulse control scale increase with AUDIT score(r>0.4). Medication noncompliance was significantly associated with hazardous alcohol use. The amount and duration of cigarette, coffee, cola use are significantly higher in hazardous drinking group. The most frequent reason for alcohol use is to go along with the group.

Conclusion£ºThe hazardous alcohol use in schizophrenics may affect clinical outcome, prognosis and their psychosocial adaptation. Therefore, it is important to assess and intervene drinking problem in a schizophrenic subgroup showing hazardous alcohol use.
